I know i did upload a version for about 2 minutes last night that claimed it was version 4.0, but that was a typo.

There is a small possiblity you might use a ISP with multiple proxy servers and one had an old copy.

I know when I moved hosting providers once I had this neat thing happen were every second image I downloaded from my site was broken - It seemed two different proxy servers were dealing with my requests and one had the new hosting provider's ip and the other had the old hosting providers ip.  Very strange.</description>
